Membership In The Chapel

If you wish to be a member of the chapel and entitled to burial from it, fill out a registration form. These are to be found on the table in the vestibule and may be left at the bookstore or brought to the sacristy.

Please be sure to notify (by the same form) of any change of address or phone number as well.
